Form Submissions
Form Submissions gives you a centralized overview of all responses submitted through forms, quizzes, and surveys across your platform. This includes contact form messages, applications, lead forms, quizzes, surveys, and any other data collected via form-based widgets.
From this view, you can review individual submissions, access attached files, navigate to the related contact profile, and export responses for further processing or reporting.
Navigating to Form Submissions
To access form submissions, go to Contacts > Form Submissions
This opens the Form Submissions view, where all collected responses are displayed in one place.
Overview of the Form Submissions View
The Form Submissions view is divided into two main areas:
- Left Panel – Submission List & Filters
- Main Panel – Submission Details

Left Panel – Submission List & Filters
The left-hand side shows a list of all form submissions, ordered chronologically.

Here you can:
- See the contact name and email (if available)
- See the submission date
- Identify which form, quiz, or survey the response belongs to
- Quickly select a submission to view its details
At the top of the panel, you can filter submissions by Form Name:
- All – Shows submissions from all forms, quizzes, and surveys
- Specific form – Shows only submissions from the selected form
💡 Note
The name shown in the filter is defined inside each individual Form, Quiz, or Survey widget. Clear naming makes filtering and analysis much easier.
Main Panel – Submission Details
When you click a submission in the left panel, the full submission details appear in the main panel.

Here you can:
- View all submitted fields and answers
- Read free-text messages (for example from contact forms)
- See uploaded files or attachments (if the form allows uploads)
- Review structured answers from quizzes or surveys
- Click User profile to open the related contact profile in your CRM
This makes it easy to move from a single submission to the broader contact context, including activity history, tags, memberships, and automations.
Accessing the Contact Profile
Each submission is connected to a contact (if an email address was provided).
From the submission view, you can:
- Open the contact profile directly
- See previous interactions, purchases, tags, and activity
- Continue communication or follow-up from a single place
This keeps form responses fully integrated with your CRM rather than isolated messages.
💡 Recommendation
It's highly recommended to require email address fields in all forms. Email address is the unique identifier for contacts, members and subscribers in the platform.
Downloading Form Submissions
You can export form submissions for offline review, reporting, or sharing.

How exporting works
- You can download:
- All form submissions
- Submissions from a specific form
- The export is prepared automatically
- A downloadable link is sent to your admin email address
- The file is delivered in a clean, easy-to-handle format (.csv)
This is especially useful for:
- Processing applications
- Reviewing survey or quiz results
- Sharing responses with a team
- Archiving lead or contact form messages
💡 Tip
Exports are ideal when you want to work with data outside the platform or combine form results with external analysis tools.
Email Notifications for Form Submissions
Each Form, Quiz, or Survey widget can be configured to send email notifications when a submission is received.
When email notifications are enabled in the widget:
- You receive an email for every new submission
The email includes the full message and submitted content
A direct link is included to open the submission in Contacts > Form Submissions
From there, you can immediately:
- Review the full submission
- Access any attached files
- Navigate to the related contact profile
This makes it easy to respond quickly to important inquiries without needing to manually monitor the platform.
💡 Tip
Use email notifications for:
- Contact forms
- Sales inquiries
- Applications or time-sensitive submissions
For high-volume forms (such as surveys or quizzes), you may prefer to review submissions directly in the Form Submissions view instead of receiving an email for each response.
